Communicate With Our Care Team

To send a message to our ONS care team:

  1. Log into your Patient Portal account.
  2. Click on Messages in the banner.
  3. Click Compose Message on the left side of the screen.
  4. In the To field, begin typing in the name of the desired group.
    • For example, if you would like to send a message to Dr. Miller’s team, begin typing in “Miller” in the To field, and Dr. Miller’s team should populate as ONS – Dr. Miller/Team.
      1. Please note: All of our doctors are listed in the Patient Portal.
    • For a Medical Records Request, please fill out the form on this page to submit a request: https://onsmd.com/request-medical-records/ or send a message in the Patient Portal to ONS – Medical Records Request.
    • For Billing Inquiries, please send a message in the Patient Portal to ONS – Billing Inquiries.
  5. Fill out the message form and click send.
