Please note: Urgent Ortho Care in Greenwich and Wilton will be closed on Wednesday, December 24, and all locations will close at 3:00 pm that day. All locations will be closed on Thursday and Friday, December 25 and 26, as well as Thursday, January 1, in observance of the holiday. ONS Physical and Occupational Therapy at all locations will close at 5:00 pm on Wednesday, December 31.

X-ray and MRI Imaging Included

Orthopaedic & Neurosurgery Specialists (ONS) has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests. Please follow the instructions below to submit your request for medical records information, X-ray or MRI images. Please note, records will not be released without a request and authorization.

Submit a Request

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. If you need to confirm your applicable ONS practice address, please visit our locations page. Your request will then be processed, and you will receive an email notification once it is complete and available for download. If you have any questions, please call HealthMark at (800) 659-4035. Please note: If you request a copy of your radiology imaging on a CD, it may take up to 7–10 business days to arrive via standard USPS shipping.
