New Patients or patients who have not visited ONS in more than a year, please arrive 15 minutes prior to your scheduled appointment time to complete your registration or save time by completing the forms online with this link> New Patient Registration and Medical History form.  Also please also bring the following to your appointment:

  • Insurance card
  • List of current medications and medical conditions
  • Primary-care physician address and phone, and contact information for other physicians involved with your care
  • Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.

From Downtown Stamford

  • Go north on Atlantic Street toward Stamford Forum. Atlantic becomes Bedford St.
  • Turn right onto High Ridge Rd.
  • Turn right onto Buxton Farm Rd., then turn left onto Turn of River Rd.
  • Turn of River Rd. becomes High Ridge Park.
  • High Ridge Park is on the right. Building 5 is the 2nd building on the left.
  • Park on the lower level in the back of the building. ONS is on the 3rd floor.

Northbound

  • CT-15/Merritt Pkwy: Exit 35 (CT-137/High Ridge Rd.), go straight at the light at the end of the exit ramp then continue on Buxton Farm Rd.
  • In .7 miles, turn left onto Turn of River Rd. and continue into High Ridge Park.
  • Building 5 is the 2nd building on the left.
    • Park on the lower level in the back of the building. ONS is on the 3rd floor.

Southbound

  • CT-15/Merritt Pkwy: Take Exit 35 (CT-137/High Ridge Rd.), at end of the ramp, turn right onto High Ridge Road (CT-137).
  • In .1 miles, turn left onto Buxton Farm Rd.
  • In .7 miles, turn left onto Turn of River Rd. and continue into High Ridge Park.
  • Building 5 is the 2nd building on the left.
    • Park on the lower level in the back of the building. ONS is on the 3rd floor.

Patient Drop off and Parking

Patient-drop off entrance and handicap accessible parking are on the lower level. Look for the patient drop-off sign on the right as you enter driveway for building #5. Handicap parking and other patient parking are on lower level.
