Before you come to your appointment, please complete the appropriate Medical History Form and Registration form (New Patients only). Please spell your name exactly as it appears on your insurance card.

Note: follow up appointments do not require completion of a Medical History Form.

Step 2. Secure Patient Portal Login and Registration Form


When you register on our Patient Portal you may:

  • Update or complete your health information
  • Request a prescription renewal
  • Pay a bill online
  • Ask a question about your bill

Parents, please note: If you wish to send a message through the portal on your child’s behalf, you must create an account for your child.

If you have previously created an account with another medical practice that uses HealthKey, you do not need to create a new account. Click here and use the same User ID and Password.
