Patients who are new to ONS or who have not visited our offices in more than a year, should complete a medical history and patient registration form online via the ONS Patient Portal.
If you do not pre- register via the portal, please download and complete the two forms below and arrive 20 minutes before your scheduled appointment to complete paperwork. This will help to expedite check-in.
Please also bring:
- Current insurance card
- List of current medications and medical conditions
- Primary-care physician address and phone number and contact information for other physicians involved with your care
- Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.

Attention: Oxford and Aetna Patients – Does your insurance plan require an in-network referral?
Please check the back of your insurance card and see if there is a box that says In-network Referral Required. If so, you will need to get an insurance referral from an in-network primary care physician, pediatrician or OBGYN.
Please note, if we verify your insurance and find that you need a referral, you will not be able to be seen by the doctor until you get one from an in-network physician.
