If you are unable to keep a scheduled appointment, please notify our office as soon as possible.

Please note: Payment is due at time of service for private pay patients.

To make an appointment with neurosurgeon Scott L. Simon, MD or Gloria Cohen, MD (Tuesdays), call (203) 487-0363, Monday through Friday between 8:30 am and 5:00 pm.

Att: Oxford and Aetna Patients- Does your insurance plan require an in-Network Referral?

Please check the back of your insurance card and see if there is a box that says In-network Referral Required.  If so, you will need to get an insurance referral from an in-Network Primary Care Physician, a Pediatrician or OBGYN.  Please note, if we verify your insurance and find that you do need a referral, you will not be able to be seen by the doctor until you get one from an in-Network physician.

Patient Portal

Patients who are new to ONS or have not visited our offices in more than a year should register on our Patient Portal to complete all the necessary forms online. To join the patient portal, click here and follow the instructions to request a portal invitation.

You may also print out and complete a registration form and medical history form prior to your appointment and bring them with you.

Note: If you do not complete the forms online, or download and complete them at home, please arrive 20 minutes before your scheduled appointment time to complete paperwork.

Please also bring:

  • Current insurance card
  • List of current medications and medical conditions
  • Primary-care physician address and phone number and contact information for other physicians involved with your care
  • Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.
