Patients who are new to ONS or who have not visited our offices in more than a year, should complete a medical history and patient registration form online via the ONS patient portal. If you do not preregister via the portal, you must arrive 20 minutes prior to their scheduled appointment time to complete paperwork.

You may also print out and complete the registration and medical history forms and bring them with you to your appointment.
All patient shoud bring the following:
- Current insurance card
- List of current medications and medical conditions
- Primary-care physician address and phone number and contact information for other physicians involved with your care
- Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.

Does your insurance plan require an in-Network Referral?
Please check the back of your insurance card and see if there is a box that says In-network Referral Required. If so, you will need to get an insurance referral from an in-Network Primary Care Physician, a Pediatrician or OBGYN. Please note, if we verify your insurance and find that you do need a referral, you will not be able to be seen by the doctor until you get one from an in-Network physician.
