As mandated by the Affordable Care Act, our patients must be registered on our patient portal so that we can communicate electronically about your care. If you have not done so already, please register on our patient portal and complete or update your Health Forms before coming for an appointment.
If you do not complete the forms online, please arrive 20 minutes prior to your appointment time to complete the process in our office.
The patient portal will allow you to manage your healthcare more efficiently through electronic communications without having to make calls and leave voice messages. You can also:
- Update or complete your Health History and Registration Forms
- Request a Prescription Renewal
- Pay a Bill online
- Ask a question about your bill
Parents please note: If you wish to send a message through the portal on your child’s behalf, you must create an account for your child.
If you have previously created an account with another medical practice that uses HealthKey, you do not need to create a new account. Click here and use the same User ID and Password.
