Please register on our Patient Portal so that we can communicate electronically about your care. Once you’re registered, please complete or update your Health Forms before coming to an appointment.
Note: If you do not complete the forms online, you must arrive 20 minutes prior to your scheduled appointment time to complete them in our office or you risk losing your appointment time to the next patient.
Use the patient portal to communicate electronically and avoid leaving phone messages:
- Update or complete your health information
- Request a prescription renewal
- Pay a bill online
- Ask a question about your bill
Parents, please note: If you wish to send a message through the portal on your child’s behalf, you must create an account for your child.
If you have previously created an account with another medical practice that uses HealthKey, you do not need to create a new account. Click here and use the same User ID and Password.
