New Patients

Patients who are new to ONS or have not visited our offices in more than a year should arrive 15 minutes prior to their scheduled appointment time to complete their registration.
For your convenience, the registration form and medical history form can be printed. Please complete and bring to your appointment.

Please bring the following:

• Insurance card
• List of current medications and medical conditions
• Primary-care physician address and phone number and contact information for other physicians involved with your care
• Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.

Via I-95, Connecticut Turnpike

Going north:
- Take exit 8. Turn left at traffic light onto Atlantic Street and proceed .6 miles to Broad Street.
- Turn right onto Broad Street.
- Take the third left onto Grove Street (which becomes Strawberry Hill Avenue) .7 miles to the top of the hill.
- At light, go left onto Strawberry Hill Court. Entrance is down the hill on the left.

Going south:
- Take exit 8. Turn right onto Elm Street and continue straight to Grove Street (which becomes Strawberry Hill Avenue) .7 miles to top of the hill.
- At light, go left onto Strawberry Hill Court. Entrance is down the hill on left.

Via Merritt Parkway

Going north:
- Take exit 34. Turn right onto Route 104 (Long Ridge Road) and go 2.3 miles toward major intersection.
- Turn right onto Summer Street and continue .7 miles to Third Street.
- Turn left onto Third Street and proceed for .5 miles (crossing Bedford Street and Morgan Street), up the hill on Strawberry Hill Court.
- Turn right at the sign into Tully Health Center.

Going South:
- Take exit 35. Turn right onto Route 137/High Ridge Road. Proceed 2.7 miles (High Ridge Road becomes Summer Street).
- From Summer Street, travel .7 miles to Third Street.
- Turn left onto Third Street and proceed for .5 miles (crossing Bedford Street and Morgan Street), up the hill to Strawberry Hill Court.
- Turn right into entrance.
